Changes in Medical Cost Composition by Hospital Size after the Benefit Coverage Extending Policy

In this study, we investigate the differences in medical cost composition by hospital size, especially focusing on the rate of uncovered medical service costs after the health insurance benefit coverage extending policy that had been enforced in 2014, using National Health Insurance Services DB and Medical Cost Survey on National Health Insurance Patients. The results showed that the rate of uncovered service costs increased by time for the groups of small sized hospitals, whereas the rate declined by time for bigger sized hospitals. This pattern still appears when using the rate of specific uncovered service costs compared to the total costs of covered services, which was predicted to decrease by year as benefit coverage extends. Next we used generalized linear models to see if this pattern appears statistically significant by including the interaction term of hospital size and year effects. After controlling for hospital level characteristics, the rate of uncovered service costs increased as the year passes, for smaller sized hospitals. This is an empirical finding that reflects the changes in medical treatment behavior due to the market change, after the benefit coverage extending policy had intervened.
